How to Register
If you live within our catchment area, you are free to register with Spencer Street Surgery.
To register as a patient of the Practice, you may apply yourself or on behalf of any person unable o make such an application themselves.

To register in person - bring along your signed medical card if you have it, or in the case of a new baby the form obtained upon baby’s birth. If you do not have either our reception staff will give you a registration form to complete.
To register online - please complete the following forms (dependant on your age) and send them along with a copy of your ID to firstname.lastname@example.org
New Patient Registration Form
You will be offered an appointment for a new patient health check.
We do ask all patients for proof of identification as you register with the Practice. The receptionists will ask you for two forms of ID, to include photo ID (passport/driving licence) and proof of address (eg: recent bank statement or utility bill). If you are unable to provide proof of identity then completion of the registration form is adequate and we will contact you if we encounter any problems with your registration on the NHS Spine.
If patients register with us with no fixed abode, they will be registered to the address of the Practice and if there are no contact numbers available for them, they will be required to attend the practice on a regular basis to collect any correspondence/results etc.
